Overall procedure

Unstable Housing Intervention Procedure

Team will implement Board Policy 507: Homeless Students

Academic Intervention Procedure

  1. Concern is raised and referral made.
  2. Director of Learning and Teaching forwards concern to Special Education Director. 
  3. Special Education Director makes determination: Does this need special education attention?
    • If yes, Special Education Director becomes Student Success Coordinator for that case and continues with process.
    • If no, Special Education Director loops in Student’s Advisor, who then becomes Student Success Coordinator. 
  4. Student Success Coordinator (either Special Ed. Director or Student’s Advisor) then creates a sub folder within the Student Success Team: Academics drive with the student’s name. 
  5. Put a copy of the referral in that folder. 
  6. Copy this form: 2. SST: Academics Coordination Form to the student’s folder, and begin the Coordination Work using that form to guide you.

          Communication Through Behavior Intervention Procedure

          1. Concern is raised and referral made.
          2. Director of Learning and Teaching forwards concern to Special Education Director. 
          3. Special Education Director makes determination: Does this need special education attention?
            • If yes, Special Education Director becomes Student Success Coordinator for that case and continues with process.
            • If no, Special Education Director loops in Director of Culture & Climate, who then becomes Student Success Coordinator.
              • If it is more appropriate to the expressed need that the Coordinator be the Justice Coordinator and not the Director, that determination can, of course, be made.
          4. Student Success Coordinator (either Special Ed. Director or Director of Culture & Climate) then creates a sub folder within the Student Success Team: Culture and Climate drive with the student’s name. 
          5. Put a copy of the referral in that folder. 
          6. Copy this form: 2. SST: Culture & Climate Coordination Form to the student’s folder, and begin the Coordination Work using that form to guide you.

                      Social / Emotional Needs Intervention Procedure

                      1. Concern is raised and referral made.
                      2. Director of Learning and Teaching forwards concern to Justice Coordinator. 
                      3. Justice Coordinator makes determination: Does this need special education attention? 
                        • If yes, Special Education Director needs to be notified. The Director can decide if they want to be a part of the team, or just want to be kept in the loop.
                      4. Student Success Coordinator (Justice Coordinator) then creates a sub folder within the Student Success Team: Justice drive with the student’s name. 
                      5. Put a copy of the referral in that folder. 
                      6. Also put a copy of the CARE request form in folder (if there is confidential information, Justice Coordinator can use judgment as to keep it in confidential folder).
                      7. Copy this form: 2. SST: Justice Coordination Form to the student’s folder, and begin the Coordination Work using that form to guide you.
